Komeiji Koishi (Hopeless Masquerade verison). She is a character you fight on story mode of Hopeless Masquerade (she first appeared in Subterranean Animism)
The brooch on koishi's shoulder is supposed to be her third eye; The bows on her shoes are supposed to be the hearts (from her third eye) on her shoes.
I tried to get as close to the reference picture that I could, that's why I chose these eyes.
