Talmar, if you aren't writing it in Hebrew, it's up to you. I've seen things written all sorts of ways for transliteration. Just one thing... second word, atah might be better, cause the way you spelled it, I'd probably not pronounce it right. Everything else seems kay.
And lessee...

Hear o Israel, the Lord your G-d, the Lord is One
Instead of using the name of G-d, I used Hashem, which means "the name", and is used by traditional Jews, who do not use YHVH except in prayer (even though the way it is pronounced is not the way the name is pronounced, but rather the pronunciation for the word that means Lord)
