Are you sure "letting you die" wasn't a part of some plan? I've seen places in zOMG! (mostly SS/EB) where letting a crew member get aggro and die from a mob (and then reviving them immediately after) is a beneficial course of action. Still, if they were, it's awful rude of them not to clarify.
I've stopped playing SS/EB-- it's too long of a setup (with the ring switching and everything) for something that takes forever to complete and fails more often than not. That being said, the coliseum is a godsend.
But I guess I'd have to say the worst thing I see on zOMG! are crew members who don't listen to their leader, especially if the leader is polite and takes time to explain everything. Whether it's a person who keeps stepping away from the group during a papa saw instance, or someone who insists on arguing against "leader opens" in SS, or simply someone who sits there and insults the leader's advice like he knows better. It just gets me mad.

It might actually have been, and if indeed it was, I wouldn't have known that. I wish they had clarified, if that was the plan. ; A ; It would have saved me a lot of frustration.

As for the third tidbit, I agree whole-heartedly.
Too many times have I gone into SS/EB leading a crew, only to have to deal with that stuff.
I have my own little set of rules and the way I do things in SS/EB {well. perhaps not 'my' way of doing it, in the sense that I made up the tactic.}, and it's gotten my crew through the entire ordeal. I tell them as nicely {and firmly} as I can without sounding too bitchy about it, and still I get arguments about it. :/
I don't see why it's so hard to keep a single opinion to oneself. It's only for one run, and at least there's gold in it. neutral
