Sounds like you have the same type of Death Note I've got Serenity... with all the rules and typos and whatnot... though mine came with the CD and it was supposed to have a feather pen but it didn't... [stupid EBay] And the pretty clear page in the front comes out of mine too...

And as for the Shingami abstinence rules... despite being obvious fodder for the immature... they do raise the question of how new Shinigami come to be... but I suppose that's another discussion entirely... [me and my crazy theories...]

I dont have one yet im going to get one sometime soon hopefully because i think they sell them at either hot topic or fye. and i would love to have one at school and take it out randomly and write all the people i hate in it. lol and see peoples reaction when they see it. that would be a blast. rofl

Better than mine...my death note gots names written on, but those are from different characters, like I got Rem's writing, Light's writing, and Mikami's writing. The cover's still Misa's note though...I purposely chose that.

I got mine from a little Japanese shop at Discover Mills (a mall by me.) It was only $13 and it was a little smaller than the other one they had (the one that comes with an L necklace.)
It's so awesome, I've already wrote down about two pages worth of names. It has all the rules in it and a few of Light's victims, and Light himself :O. And my friends have tried to steal it, 'specially my ex.
Funny thing, for the first two weeks I had it, I brought it to school with me every day. First name I wrote in it was when I was in orchestra and I snapped. Took me two weeks to do it xD.
