I think it's from some form of H-game, judging by the art style. Might just be a random piece of art, though...

I'm actually trying to remember the name now... I'm tearing my room apart looking through all my manga, lol. I can't place it T.T I think its one of the ones my friend is borrowing. But I clearly remember the scene. This is gonna drive me crazy til I find the book

EDIT: The older male is sick and the younger one is trying to cheer him up, this is his... sick... demented... twisted way to tell the younger boy that he's ok, lol
