Im trying to hard
To find out what means the most to me
Happiness; Joy; Health, or
A simple Far off Memory

It is who you are
What is truly inside of you
What you think, hope for, wish for
Every little thing you do

Its creating to priceless puzzle,
but the pieces are dissapearing, one by one
Help me find them
Or else this puzzle will never be done

I wanted to line the pieces up,
all of yours, and all of mine
to wrap the memories,
mold them together like twine

A scattered dream
is like a far-off memory
and a far-off memory is
like a scattered dream, you see

One day you may dissappear...
But a memory lasts forever
As long as your in it
So that we can remain together

Sounds like a song. I could picture it as like a theme. I mean, you've heard Utada Hikaru's work. It's all emotional and stuff. I liked it ^^
4/5 Maybe work on the last part some, the rhyming seems off...might be just me...
I have a suggestion, if it doesn't branch off from your original meaning of the poem:
One day you may disappear...
But a memory lasts forever.
As long as you are in it,
Then we'll remain together.
Just an idea =P
