Sailor Cosmos was a character who played an important part in the Stars arc of the Sailor Moon manga, where she was revealed to be the true form of Chibi Chibi who had showed up out of the blue one day. It's unclear of who exactly Sailor Cosmos is, whether she is the final form of the current Sailor Moon or a descendant of Usagi's.
